Which adhesive would best for glueing the bulk fabric to the old form with some of foam not completely cleaned off?

None. There is no adhesive that will fix this problem. The old foam is disintegrating so attempting to glue material back to it won't work. You will need to scrape and wirebrush all the old foam off and replace with new foam parts.
